Please take the time to read over our policies prior to booking online services. Document links for Covid Consent Policy and New Client/ Returning Client intake forms will be emailed to clients after services are booked. All necessary forms must be submitted prior to scheduled appointment time.

Covid form must be signed no sooner than 24hrs prior to your scheduled appointment with Chakras Royale. You must meet the requirements and agree to the following terms:

I currently am not or have not been experiencing any COVID-19 symptoms which may include but not limited to fever or chills, cough, shortness of breath, fatigue, muscle or body aches, loss of taste or smell, sore throat, congestion or runny nose, nausea/vomiting, diarrhea.

  * I or anyone in my home have not been diagnosed with COVID-19 in the last 30 days. 

  * I have not traveled outside of the country in the last 30 days.

  * I have not knowingly been exposed to anyone with COVID-19 in the last 30 days.

 I understand that, because massage therapy work involves maintained touch and close physical proximity over an extended period of time, there may be an elevated risk of disease transmission, including COVID-19 whether symptoms are present or not. Chakras Royale does not require that a facemask be worn by the client during the massage session as the therapist may prefer to wear a facemask within 6ft of he client. By signing this release, I hereby waive and release my therapist from any and all liability (past, present, and future) relating to massage therapy and bodywork. By signing, I agree that I am providing accurate health information to the best of my knowledge and I also agree that I have fully read and understood this Covid-19 Statement.

 I understand that massage therapy is provided for stress reduction, relaxation, relief from muscular tension, assists with pain management, improvement of circulation, mobility and holistic wellness. I agree and understand that treatments are for therapeutic purposes only and should NOT be used if the person who will receive the treatment is on any medication that would be expected to weaken bone structure and/or connective tissue or lead to enhanced bleeding; have edema due to chronic heart failure or kidney failure; have an inflammatory condition in the acute stage; have acute phlebitis and/or deep venous thrombosis; have had an acute trauma or recent surgery; are in the first trimester of pregnancy, or where treatment is otherwise contraindicated. Clients that wish to book a treatment on his or her own behalf should consult a physician if uncertain as to whether a treatment is appropriate. I agree that no one other than myself or an established client approved by Chakras Royale may receive treatment during my scheduled appointment. I understand that massage therapists are not physicians, and that massage therapy should not be construed as a substitute for medical examination and/or treatment provided by a licensed physician. I understand that massage therapists cannot diagnose any physical or mental illnesses, that they do not prescribe any medical treatments or drugs/medications, and that they do not perform any spinal or skeletal manipulation. I understand that Broad-spectrum CBD hemp extract preserves the full range of cannabinoids, terpenes and flavonoids that the hemp plant naturally provides, enhancing the efficacy of the CBD through the entourage effect and that this product is not intended to diagnose, treat, cure or prevent any disease.  I understand that while botanical and essential oils are natural plant extracts, there is still the possibility of allergic reactions and skin irritation in certain individuals. Chakras Royale Holistic Wellness LLC is not responsible for any allergic reactions if at any time irritation occurs from the use of any products and client is advised to discontinue use of the products immediately.

Expected Conduct: I understand that massage is entirely therapeutic and "non-sexual in nature". The therapist reserves the right to immediately end the session if any sexual advances, comments, or jokes involving a sexual nature occur during the massage session. You will be required to pay the full price of your session and will not be allowed to book any future sessions at Chakras Royale Holistic Wellness. I understand this is a professional therapeutic relationship with my therapist. The therapist reserves the right to immediately end your session if you refuse to be draped during the session. You will be required to pay the full amount of your session. If you have a card on file with us, you authorize Chakras Royale to charge your card for the session if you break any of the policies of Chakras Royale Holistic Wellness. The breast and genital area will not be massaged under any circumstances.

Draping: I fully agree and understand that I must be draped during sessions/treatments. I may choose to leave on or remove as much clothing as needed for my level of comfort, refuse any massage methods, or stop the massage at any time. Only the area being massaged will be undraped and the areas undraped will be secure to ensure there is no indecent exposure to the clients' private areas.  If undraping the client's gluteals is significant in the treatment, I understand that it is part of therapy, and I always have the right to refuse treatment of the gluteal area or request that the gluteals be massage over the sheet. I understand that the therapist will proceed to massage the gluteals under the sheet if the client completely disrobes or will proceed to massage gluteals over the sheet if the client disrobes down to undergarment unless client refuses treatment of the gluteal area. The client is informed that they always have the right to terminate treatment at any time, and the right to alter the therapist's pressure if it is causing you (The client) discomfort. The therapist reserves to right to immediately end your session if you refuse to be draped during the session and you will be required to pay the full amount for your session. If you have a card on file with us, you authorize Chakras Royale to charge your card for the session if you break any of the policies of Chakra Royale Holistic Wellness LLC.

Confidentiality: The discussion between the massage therapist and the client is confidential. The client may or may not choose to talk during the massage. The client understands that their personal health information and any other client records maintained by the therapist are private and confidential. The client understands that their information will not be shared with any third party without their prior written consent or a subpoena or court order. Client records and private health information cannot be shared with spouses, significant others, or family members without written consent from the client. We are happy to listen to your conversation and share our professional expertise or respectful opinion . We prefer to not discuss topics of political, religious, or sexual nature. There are times where a client may have emotional responses during a session such as crying, sadness, or have a moment where a client feels a rush of powerful emotion. We want our clients to know that it is absolutely ok to express those feelings in our safe, nonjudgmental environment. In addition, you may request privacy, ask that the therapist pause the session and be present with you during that time, or you may choose to end the session. You are always in control of your massage session.

Cancellation Policy/No-Shows/Tardy: We have a 24-hour cancellation policy. This allows the opportunity for someone else to schedule an appointment. If you are unable to give us 24 hours advance notice you will be required to pay the full amount of your session. Clients who have a positive COVID reading within 24hrs of their scheduled appointment will be required to reschedule no sooner than 14 days after positive COVID reading. You will be required to pay the full amount of your session which will be credited towards your rescheduled appointment. All notifications must be made via email. We will not accept cancellations from anyone else other than you. Anyone who either forgets or consciously chooses to forgo their appointment for whatever reason will be considered a “no-show.” You will be charged the full price of the scheduled session for their “missed appointment". Clients that are late for their scheduled appointment time will still be granted the remaining time of their session. Your session time will not be extended, and your session will end on time so that other clients following your appointment are not penalized. If you have a card on file with us, you authorize Chakras Royale to charge your card for the session if you break any of the policies of Chakra Royale Holistic Wellness LLC.

 

Intake Forms: Please note that we cannot begin your session if your intake form and any other necessary paperwork are not completed before your appointment. Some of your forms will take time to fill out and your session appointment may be canceled as a result of not having your paperwork completed before the session. Your session time will not be extended, and your session will end on time so that other clients following your appointment are not penalized. Failure to complete paperwork upon therapist arrival for mobile services will result in cancellation of appointment and you will be required to pay the full amount for your session. If you have a card on file with us, you authorize Chakras Royale to charge your card for the session if you break any of the policies of Chakra Royale Holistic Wellness LLC.

 

I hereby state that, to the best of my knowledge, my answers to the above questions are correct. I agree and consent to assessment and treatment. I understand that my personal health information will be collected. I understand that all information that I provide will be kept confidential unless required by law. I understand and consent that my medical information may be shared by the various care providers involved in my care and treatment. Treatments may be covered by extended health care plans. I understand that it is my responsibility to confirm the exact details of my coverage.
